Osteonecrosis of the femoral head (ONFH), also known as avascular necrosis (AVN), is a debilitating orthopedic condition characterized by the death of osteocytes and bone marrow components due to an interruption of the blood supply to the femoral head. The etiology of non-traumatic ONFH is multifactorial, with over 40 identified risk factors. The most prevalent are chronic alcohol abuse and corticosteroid use. Literature reports indicate that 10% to 74% of ONFH patients have a history of significant alcohol consumption, with cumulative intake exceeding 150 liters of pure alcohol (equivalent to approximately 400 ml of alcohol weekly) increasing the risk. Steroid-induced ONFH has an incidence ranging from 18% to 46%, showing a strong correlation with the dose, duration, and route of steroid administration, particularly high-dose, short-term courses. A cumulative prednisone dose exceeding 2000 mg is often cited as a threshold for a significantly increased risk. Other recognized etiologies include systemic lupus erythematosus, sickle cell disease, Gaucher's disease, hyperlipidemia, pancreatitis, deep-sea diving (caisson disease), coagulopathies, and organ transplantation. Idiopathic ONFH, where no specific cause is identified, also constitutes a significant proportion.

ONFH poses a worldwide orthopedic dilemma due to its high propensity for progression and severe functional impairment. Approximately 80% of untreated patients will experience femoral head collapse within 1 to 4 years, leading to significant disability. The profound impact on patient quality of life, coupled with the limitations of current treatment modalities, underscores the critical need for effective joint-preserving strategies, especially in the younger demographic prevalent in regions like China. The objective of early diagnosis and a tailored, staged approach is to arrest disease progression, prevent collapse, preserve the native hip joint, and restore pain-free function, thereby delaying or avoiding the need for THA.

Surgical Anatomy & Biomechanics

A thorough understanding of the vascular anatomy and biomechanics of the femoral head is fundamental to comprehending the pathophysiology of ONFH and guiding surgical intervention.

Vascular Supply of the Femoral Head

The femoral head receives its primary blood supply from the medial femoral circumflex artery (MFCA), a branch of the profunda femoris artery. The MFCA typically gives rise to ascending cervical branches that travel superiorly along the femoral neck, ultimately forming the lateral epiphyseal arteries. These lateral epiphyseal arteries pierce the capsule at the retinacular folds (Weitbrecht's retinaculum) and supply the superior and anterior aspects of the femoral head, which constitute the critical weight-bearing zones. The inferior and posterior aspects receive contributions from the inferior gluteal artery and the obturator artery through the artery of the ligamentum teres, though this contribution is often less significant in adults and typically insufficient to sustain the entire femoral head in the event of MFCA compromise. The vulnerability of the lateral epiphyseal arteries to injury or thrombosis, especially at the femoral neck-head junction, is a key factor in the development of ONFH. Any disruption to these terminal end-arteries can lead to localized ischemia and subsequent osteocyte death.

Femoral Head Structure and Biomechanics

The femoral head is a spherical structure composed of a thin cortical shell surrounding a robust network of trabecular bone, all encased by articular cartilage. The subchondral bone, a specialized layer immediately beneath the articular cartilage, plays a crucial role in load transmission and shock absorption.
The hip joint is a congruent ball-and-socket joint designed for stability and mobility, capable of bearing immense loads. During daily activities, forces across the hip joint can range from 3 to 7 times body weight. These forces are primarily transmitted through the superior and anterior aspects of the femoral head, corresponding to the typical location of necrotic lesions.

In ONFH, the death of osteocytes weakens the trabecular bone within the necrotic zone. This necrotic bone, devoid of viable cells, loses its capacity for repair and remodeling. Under repetitive physiological loading, microfractures begin to accumulate within the weakened subchondral bone. Initially, the intact overlying articular cartilage may mask the underlying bone pathology. However, as the necrotic segment becomes structurally incompetent, it eventually succumbs to the compressive forces, leading to a subchondral fracture (the "crescent sign" on radiographs) and subsequent collapse of the articular surface. This collapse disrupts the smooth articular contour, leading to incongruity between the femoral head and acetabulum, accelerated cartilage wear, and secondary osteoarthritis. The process of "creeping substitution," where viable bone attempts to replace necrotic bone, is often inadequate or too slow to prevent collapse in the face of ongoing mechanical stress.

Indications & Contraindications

The decision-making process for ONFH management is complex, necessitating a comprehensive evaluation of the patient's age, etiology, systemic health, symptom severity, disease stage, lesion size and location, and the integrity of the articular surface. The primary goal of joint preservation is to arrest disease progression and prevent or delay femoral head collapse.

Staging Systems

Accurate staging is paramount for guiding treatment and predicting prognosis. While numerous systems exist (e.g., Ficat, Marcus, Steinberg, Japanese Orthopaedic Association), there is no universally adopted method. The ARCO (Association Research Circulation Osseous) staging system and the University of Pennsylvania classification are widely favored for their comprehensive inclusion of pathological, radiological, and MRI findings, along with subtyping based on lesion size and collapse extent.
