Ursodeoxycholic acid (UDCA) is a naturally occurring bile acid that is used therapeutically to treat various liver and gallbladder conditions. Its primary mechanism of action involves altering the composition of bile. UDCA is less toxic and more hydrophilic than other bile acids, meaning it reduces the cholesterol saturation of bile. This effect helps to prevent the formation of cholesterol gallstones and can gradually dissolve existing cholesterol gallstones by reducing bile's lithogenicity. It also exhibits cytoprotective effects on liver cells, protecting them from damage caused by toxic bile acids.

Clinically, UDCA is indicated for the dissolution of cholesterol gallstones in patients with symptomatic cholelithiasis who are not surgical candidates. It is also a cornerstone treatment for primary biliary cholangitis (PBC), an autoimmune disease that causes progressive destruction of the bile ducts in the liver. By improving bile flow and reducing inflammation, UDCA can slow the progression of PBC and improve liver function. Common side effects are generally mild and can include diarrhea, nausea, and abdominal discomfort. More serious side effects are rare but can include liver enzyme elevations.
