Tofacitinib is a targeted synthetic disease-modifying antirheumatic drug (DMARD) belonging to the class of Janus kinase (JAK) inhibitors. It works by selectively inhibiting JAK enzymes, which are intracellular signaling molecules involved in the immune response. By blocking these pathways, tofacitinib modulates the inflammatory cascade, reducing the production of pro-inflammatory cytokines that contribute to the pathogenesis of various autoimmune diseases. Due to its immunosuppressive effects, tofacitinib carries a risk of serious side effects, including increased susceptibility to infections (bacterial, viral, and fungal), the development of certain malignancies (lymphoma and skin cancer), and an increased risk of major adverse cardiovascular events (MACE), such as heart attack and stroke, particularly in patients with pre-existing cardiovascular risk factors.
