Propafenone is a potent antiarrhythmic medication primarily indicated for the management of life-threatening ventricular arrhythmias and the maintenance of normal sinus rhythm in patients with symptomatic atrial fibrillation or flutter. As a Class 1C agent, it works by blocking sodium channels in the myocardial cell membranes, which slows the conduction velocity and increases the refractory period within the heart's electrical system. This stabilization helps prevent the rapid, irregular heartbeats associated with various cardiac arrhythmias.
Clinical administration of Propafenone requires careful monitoring, particularly during the initiation of therapy, due to its potential for proarrhythmic effects. Common side effects may include dizziness, altered taste (metallic taste), nausea, and constipation. Because of its mechanism of action, it is generally contraindicated in patients with structural heart disease, such as heart failure or a history of myocardial infarction, and requires dosage adjustments in patients with hepatic or renal impairment.
