Colesevelam is a non-systemic, lipid-lowering agent that functions as a bile acid sequestrant. By binding to bile acids in the intestinal lumen, it prevents their reabsorption, forcing the liver to utilize circulating cholesterol to synthesize new bile acids, thereby lowering serum LDL cholesterol levels. Additionally, it is indicated as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus, likely through mechanisms involving the modulation of incretin hormones or glucose absorption.

Common side effects primarily involve the gastrointestinal tract, including constipation, dyspepsia, and nausea. Because Colesevelam can interfere with the absorption of other medications (such as fat-soluble vitamins or certain oral contraceptives), it is clinically recommended to administer other drugs at least four hours before or after taking Colesevelam to ensure therapeutic efficacy.
