Balsalazide is a prodrug belonging to the class of aminosalicylates. Upon oral administration, it passes through the upper gastrointestinal tract largely intact, reaching the colon where it is cleaved by bacterial azoreductases into 5-aminosalicylic acid (5-ASA or mesalamine) and 4-aminobenzoyl-beta-alanine. The released mesalamine acts locally on the colonic mucosa to inhibit the production of inflammatory mediators, such as prostaglandins and leukotrienes, thereby reducing inflammation in patients with ulcerative colitis.
Clinically, balsalazide is indicated for the induction of remission in patients with mild to moderate active ulcerative colitis. Common side effects include headache, abdominal pain, diarrhea, and nausea. Because it is a salicylate, patients with a known hypersensitivity to salicylates or any component of the formulation should avoid its use. Patients should be monitored for renal function, particularly those with pre-existing kidney disease, as aminosalicylates have been associated with nephrotoxicity.

من الناحية السريرية، يُستخدم بالسالازيد لتحفيز حالة الهجوع (الشفاء المؤقت) لدى المرضى الذين يعانون من التهاب القولون التقرحي النشط بدرجة خفيفة إلى متوسطة. تشمل الآثار الجانبية الشائعة الصداع، وآلام البطن، والإسهال، والغثيان. ونظراً لكونه من فئة الساليسيلات، يجب على المرضى الذين لديهم حساسية معروفة تجاه الساليسيلات أو أي من مكونات التركيبة تجنب استخدامه. كما يجب مراقبة وظائف الكلى لدى المرضى، خاصة أولئك الذين يعانون من أمراض كلوية سابقة، حيث ارتبطت أدوية الأمينوساليسيلات بحدوث سمية كلوية.
